Virtual Leader Trainings for Chronic Conditions Self-Management

Leader trainings teach CHWs how to teach SMRC programs for disease self-management, diabetes self-management, cancer thriving and surviving, pain self-management, caregiver program for family caregivers. All trainings are offered in Spanish, and some may be available in other languages as well. None of the trainings are asynchronous because the content is extremely rigorous and skills-focused. Participants will be given manuals to study and participate in a requisite number of practice teaches in order to receive certification. SMRC will ensure each applicant as key skills before accepting them to participate in the training. This training is appropriate for CHWs that wish to offer self-management resource center workshops to their clients, or if an organization wants CHWs trained to offer SMRC programs locally. SMRC training would upscale CHW skills. SMRC has done a lot of work with tribes to provide culturally competent SMRC leader trainings.

Virtual/In-Person/Hybrid: Trainings can be provided via telephone for up to 3-6 people, video for up to 12 people, and in person for up to 14 people. The online course is online via 15 sessions or in-person for 4 days.

Registration: CHWs must be affiliated with a liscensed organization to be eligible. Organizations may use the SMRC program locator to see if there are any current SMRC being offered locally, and ask to have their CHWs join that training. Also, if enough CHWs in a state want to participate in the training, they can work with SMRC to create a custom, in-person training. Organizations would need to vet SMRC training as part of a CHW job description, as it is too rigorous to be an extra-curricular activity.
