Certificate Program for Diabetes Community Care Coordinator

This program is designed for members of the diabetes care team focused on linking individuals to resources, building relationships, and supporting people with, or affected by, or at risk for diabetes and cardiometabolic conditions. Talks through the basics of diabetes (what is diabetes, different types of diabetes, complications, self-care behaviors coaching, and connecting patients with services in the community). Self-study, self-paced program with approximately 11 hours of coursework. No CE/CME credit offered. Certificate of completeion upon successful completion of coursework. Includes audio-visual presentation, closed captioning, breaks for check-your-knowledge questions, downloadable slides. Must complete all programming within 2 months from the date of registration.
